New Patient Registration

Please call our office at the location you prefer and request an appointment. We will need some basic information. Have your insurance cards in front of you.

For your convenience, we also have new patient registration forms that can be downloaded. Please complete these forms in advance and bring them with you. We will also need a written list of medications you are taking. The day of your appointment, allow at least 90 minutes for a comprehensive eye examination.
